This, along with the name "Darth" is one of the things which had been written as coming about during the Brotherhood of Darkness and the rise of Darth Bane, but then the KotOR games adopted them unaware of that backstory, to provide games that would feel familiar to those who were fans of the movies, not the EU.
As the KotOR games are very popular, and for that matter well-written and fantastic additions to the EU, the adoption of both traits, the red lightsaber and the title of Darth, is now distinctly placed as beginning with Darth Revan. The previous Dark Lord of the Sith, Exar Kun, did not use the title Darth and wielded a blue lightsaber, then a blue double-bladed lightsaber. Then at the beginning of the first KotOR game, forty years after the death of Lord Kun, you have red lightsabers standard among all Sith. Presumably, Revan either discovered a large source of red lightsaber crystals that could be used to supply his forces, or developed the technique to create synthetic crystals.
